Did you know that you can use ToolBook to create surveys and use your LMS to collect the information you collect?

The only thing that is required is ToolBook and an LMS which is able to show question level feedback in reports.

I have created a sample (download SurveySample.tbk) that you can download but will also explain what is required. You can also see it here.

The following ToolBook widgets are useful:

  • Rating (ie Likert Scale)
  • Multiple choice Question
  • Fill in the blank (especially the multiline feedback)

Here is a sample of a Rating/Likert Scale question

Survey1
 
You should name your question objects sensibly so you can identify the question in the LMS

Survey 2 - name your questions

When asking for people’s opinion it doesn’t make a lot of sense to have incorrect answers (and yes you DO want to get the negative feedback)

Since there is no correct and incorrect, then having immediate or delayed feedback isn’t really helpful either so just turn that off.

Survey 4 - Only correct answers and no feedback

Now publish the content and upload to the LMS and make it available to the relevant users.
 
The user launches the survey and answers the questions.

Survey 5 - make the survey available through an LMS

When the user has answered all questions they exit and mark the course as complete.

Survey 6 - answering the survey in the LMS
 
The users responses is now available the report section of your LMS. Some LMS allow you to export the report in PDF or Excell or similar. 

Survey 7 - Question level detail in reports in LMS

Do notice how the name of the question and the name in the report is connected: a Multiple Choice object with Object id 8 on page id 1 named Survey1 will become  Survey1__P1_8 when published to SCORM.

Thank you for a very interesting article (and perfect timing with my current project!)

I was wondering if there is an easy way that an end of course survey (or feedback form) could be submitted to a specific email instead of sending results to an LMS using ToolBook?

(I hope you see this as I dont know your email adress so I cant notify you).
Glad to help. What you are asking is essentially how one can track ToolBook content without having an LMS. It is is possible - though a little more involved than what the above describes.
In your case I would use this approach: Use the action editor to collects the answers as the user responds and then use ToolBooks Action Editors HTTP post feature to send the results to a serverside page (.net or asp or similar) which then takes care or sending the email.
